Buehler Family Chiropractic offers our patients two options for first day paperwork so it can be completed in the convenience of your own home or office, before you even arrive to our chiropractic office. The preferred option is to complete the forms online and the information is transmitted to our office automatically, eliminating the need to print the forms out at home. The second option is to print out the paperwork and fill it out ahead of time before arriving at the office.
